Medication errors are one of the most common types of error in hospitals. This module discusses the ‘route’ of the problem, by identifying key factors that contribute to medication administrations errors such as illegible orders, omission errors, and improper dosage or medication selection. Primary contributing factors to medication errors, the 5 R’s of Medication administration are reviewed and tips to decrease medication errors are discussed.
